This
application for exhibit space or sponsorship at the Compliance & Market
Conduct Exchange (the “Conference”) will
become an Agreement or contract upon acceptance by Life Office Management
Association (LOMA), a Georgia non-profit corporation and LIMRA International,
hereinafter referred to as the “Conference Organizers.” The Conference
Organizers, at their sole discretion, will accept appropriate applications by
applicants on a space available; first-come, first-served basis. Accepted
Applicants will receive a confirmation e-mail upon acceptance of this
application. To ensure the quality of Conference exhibitors and sponsors, the
Conference Organizers reserve the right to exclude Applicants that in LOMA and
LIMRA’s sole discretion deem inappropriate for the Conference. The term
“Hotel” shall refer to San Antonio Marriott Riverwalk Hotel, the official
hotel for the Conference.
Sponsorship
is not confirmed until LOMA has received a signed application with the agreed
upon fees paid in full. Unpaid sponsorships will be subject to being resold.

exhibitor fee includes two conference registrations, which does not include
hotel or related expenses.

Any
personnel from your company, beyond the two conference registrations that are
included with your exhibitor fee, must register at the regular conference
registration fees. They can register online or download a registration form at http://www.loma.org/marketconduct.asp.
The registration fees are LOMA/LIMRA Members $825 & Nonmembers $1,295 per
person by October 3, 2008.

Listing
as a sponsor on pre-show marketing materials (registration brochure),
reaching more than 5,000 people. (A signed contract and full payment for the
chosen sponsorship must be received by LOMA by July 15, 2008 to be listed in
registration brochure).

for 2 people from your company. The individuals will be officially
registered and listed as delegates and invited to attend all conference
events. Additional personnel must register at the regular conference
registration fees and may register online at http://www.loma.org/marketconduct.asp.

Materials Sponsor Fee:
$2,000
Provide
us with your branded item(s) and we will stuff them into the attendee bags. You
can provide small items, such as mugs/stainless steel mugs, pens/highlighters,
pads, Post-It Notes, luggage tags, USB memory sticks/flash drives, or 2009
calendars. If you have an item that is not listed that you would like to
provide, please contact us to discuss. The cost for this is $2,000 for each item
that you choose to provide. This fee does not include exhibit space and does not
include your conference registration(s).
CANCELLATION
BY EXHIBITOR/SPONSOR
Cancellation
of sponsorship or exhibit space and/or registered personnel must be received in
writing. Any exhibitor/sponsor cancellations received on or before September 5, 2008 will be subject to a
cancellation fee of 50%. No refunds will be made for cancellations received after
September 5, 2008.
CANCELLATION
BY CONFERENCE ORGANIZERS
In
the event that this scheduled Conference or event is canceled by the Conference
Organizers for any reason, the Conference Organizers shall refund any
exhibitor/sponsor fees that have been paid by the exhibitor/sponsor. Under no
circumstances, however, shall the Conference Organizers be liable to the
exhibitor/sponsor for any other expenses, including, but not limited to,
airfare, hotel or any other travel-related expenses incurred by the
exhibitor/sponsor.
SPECIAL
ASSISTANCE
In
compliance with the Americans with Disabilities Act, all personnel needing
special assistance should contact LOMA with their requests in writing.
EXHIBITOR/SPONSOR
RULES
-
Exhibitor/Sponsor
assumes responsibility and agrees to indemnify and defend the Conference
Organizers and the Hotel and their respective employees and agents against
any claims or expenses arising out of the use of the exhibition premises.
Neither the Conference Organizers nor the Hotel maintains insurance covering
the exhibitor’s/sponsor’s property, and it is the sole responsibility of
the exhibitor/sponsor to obtain such insurance.

Exhibitors/Sponsors
are not to imply LOMA or LIMRA International’s endorsement of their
products and/or services, nor use LOMA or LIMRA International’s logos or
names without LOMA and LIMRA International’s written consent. Selection as
an exhibitor, sponsor, or speaker does not in any way imply an endorsement
by LOMA or LIMRA International of the exhibitor’s, sponsor’s, or
speaker’s products and/or services.

Exhibitors/Sponsors
are prohibited from scheduling parties, receptions, product demonstrations,
or any such social events that conflict with official conference hours. If
you are planning a company-sponsored function of any type, the Conference
Organizers must be informed of the date, time, and location.
-
Exhibitors/Sponsors
are prohibited from sharing, subletting, or assigning their exhibit space or
any part thereof, to third parties. Only one company per exhibit space
allowed.
-
Exhibitor’s
tabletop display or similar sized display must fit in assigned exhibit space
without obstructing the view of other exhibitors. Exhibitors may be required
to adjust display.
-
Exhibits
that include the use of any noise making equipment (musical instruments,
videos, etc.) must have Conference Organizers’ prior approval. Display
decorations and materials must be compliant with all Hotel and state
requirements. Volume, including sound and/or size of exhibit, may not
disturb other exhibits.

All
activities and exhibit personnel must be confined to the allocated exhibit
space. All individuals working the exhibit or associated with the company
must be registered and wear a badge at all times
-
All
charges for guestrooms or other expenses incurred by exhibitor/sponsor, will
be payable by exhibitor/sponsor directly to the Hotel, as appropriate.
Electronic
Records & Signatures Terms & Conditions
By
agreeing to the terms of this agreement and registering as a sponsor or
exhibitor, I understand and agree that I am submitting this application for
exhibit space/sponsorship by using my electronic signature and that my
electronic signature is as legally binding as a manual signature on paper. These
Electronic Records and Signatures Terms and Conditions govern my relationship
with the Conference Organizers and apply to all communications with the
Conference Organizers regarding this application and Agreement for exhibit
space/sponsorship.
By
agreeing to the terms of this agreement and registering as a sponsor or
exhibitor, I agree and consent that information related to this application and
Agreement may be provided to me electronically. I agree and consent to be bound
by the terms and conditions of the Agreements, this application, and other
related information provided electronically. I consent to the use of electronic
records and electronic signatures in conducting business with the Conference
Organizers and their service providers, including processing any payment
information supplied by me.
I
consent to provide the Conference Organizers with my current e-mail address and
to notify the Conference Organizers of any changes to my e-mail address by
calling 770-984-6453 or submitting my new e-mail address from my old e-mail
address, with an effective date of the change.
I
understand that in order to electronically receive agreements, applications, and
other information, I will need a compatible operating system, Web browser, and
e-mail application, and I will need access to a printer or the ability to
download information to keep copies of such documents for my records. The
operating systems, Web browsers, and e-mail applications identified at the end
of these Terms and Conditions are compatible.
I furthermore understand that I am indicating that I have the capability
to access the agreements, applications, and other information and download or
print copies for my records. I understand that LOMA’s electronic documents are
compatible with the following operating systems, Web browsers, and e-mail
applications:

I
agree that I may contact LOMA at 770-984-6453 or by e-mail at mcclough@loma.org

electronic signatures. I understand and agree that I may obtain a paper copy of
the agreements, applications, and other information at anytime by contacting
LOMA by telephone at 770-984-6453. LOMA will/will not charge a fee for any paper
copy.
I
agree and consent that I may withdraw my consent to receive electronic records
and use electronic signatures by contacting LOMA at 770-984-6453. However, I
agree and consent that if I do withdraw my consent, LOMA may cancel my online
services or require me to submit a written request for paper copies.

AGREEMENT
The
undersigned, on behalf of the Applicant, its employees, agents, and contractors,
agrees to comply with the rules set forth in this document by the Conference
Organizers. All correspondence will be sent to mcclough@loma.org.
This
document contains the entire agreement between the parties and supersedes any
prior agreements. The terms of this document may not be changed except in
writing and signed by the parties.
