Ed Reps administering LOMA's Education & Training programs at their company can print and fax these forms directly to the Office of the Registrar. These files are saved in a PDF format. To view the files, you must have Adobe Acrobat Reader installed on your computer. If you don't already have it, you can download a free copy from Adobe's website.

• Award Replacement Order Form
• Order a replacement award for any LOMA education program.
• Fee Transfer Form
• Purchase a fee transfer for students who were unable to sit for their examination for any reason.
• Ed Rep Change Form
• Notify LOMA of a change in the Ed Rep at your organization.
• Promotional Material Order Form
• Complete and submit this form to PBD to order promotional materials, brochures, posters, and more!
• Secondary Location Form
• Establish a Secondary Location number for administering LOMA exams in a branch office, or change the Proctor at a Secondary Location.
• Corporate Admin Change Form
• Add a new Corporate Admin or change the Corporate Admin for LOMALearn.
