This application for exhibit space/sponsorship at the DI & LTC Insurers' Forum (the “Conference”) will become an agreement (the “Agreement”) upon acceptance by Life Office Management Association, a Georgia non-profit corporation (“LOMA”).  LOMA will accept appropriate applications on a space available first-come, first-served, basis.  Accepted applicants will receive a confirmation e-mail upon acceptance of this application. To ensure the quality of Conference sponsors, LOMA reserves the right to exclude Applicants that LOMA, in its sole discretion, deems inappropriate for the Conference.

Sponsorship is not confirmed until LOMA has received a signed application with the agreed upon fees paid in full.  Unpaid sponsorships will be subject to being resold.


Exhibit and Sponsor Opportunities

Exhibitor Fee: $2,700
One complimentary conference registrations is included with each display reserved. One additional individual may register for the conference at the rate of $495; each additional person can register for the standard rate of $975 per person. The exhibiting organization may elect to register additional staff members at any time prior to the conference.

SPACE ASSIGNMENTS – Space is limited to tabletop displays or similar sized displays.  Each exhibiting organization will receive:

  • 6-foot skirted table

  • Electrical outlet

  • One conference registration

  • One-time use mailing list (Your choice - three weeks prior to or three weeks following the event.)

  • Listing in the on-site conference material

  • Listing on vendor flyer included in conference registration packets

  • Recognition on general session walk-in slides

Display selections are not confirmed until the Conference Organizers have a signed contract. Spaces will be assigned by the Conference Organizers based on the order in which the contracts are received. Unpaid display spaces after August 22, 2008 will be subject to being re-sold to other clients.

Displays should be assembled no later than 1 p.m. on Wednesday, September 24.  Displays may be dismantled and removed at the conclusion of the conference sessions on Thursday, September 25 or at the conclusion of the 8:45 a.m. refreshment break on Friday, September 26.

Displays will be located in the area immediately adjacent to the primary meeting rooms for the conference.  While there are no official hours for the exhibits, exhibitors are encouraged to have staff at their display during the following functions and times (tentative):

Registration: Noon - 2:00 p.m.

Refreshment Break: 3:15 – 3:45

Reception: 5:30 – 6:30 p.m.

 

Thursday, September 25, 2008

Continental Breakfast: 7:30 – 8:30 a.m.

Refreshment Break: 10:00 – 10:30 a.m.

Lunch: Noon – 1:15 p.m.

Refreshment Break: 2:15 – 2:45 p.m.

Refreshment Break: 3:45 – 4:00 p.m.

Exhibitors are invited to sign up for the dine-around event taking place this evening.  Sign up will be available at the registration desk

 

Friday, September 26, 2008

Continental Breakfast: 7:15 – 8:00 a.m.

Refreshment Break and exhibitor prize drawing:  8:45 – 9:15 a.m.

It is in the best interest of the exhibitors to have a representative at their display during these periods. Each exhibitor will receive two conference registrations for use by the exhibit staff to attend the entire conference and all functions associated with the conference.

SPONSOR OPPORTUNITIES:
Sponsorships are not confirmed until the Conference Organizers have a signed contract. Spaces will be assigned by the Conference Organizers based on the order in which the contracts are received. Unpaid sponsorships after August 22, 2008 will be subject to being re-sold to other clients.

Sponsors of the below events will automatically receive the following:

Reception Sponsor: $9,000 – Includes exhibit booth
Lunch Sponsor: $6,250 – Includes exhibit booth
Breakfast Sponsor: $5,250

  • One exhibit space with one electrical outlet - the exhibit area will be host to a reception Wednesday evening, continental breakfast each morning and an extended break on Wednesday and Thursday

  • Registration for 2 people.  Additional personnel can be registered at the rate of $ 975. The individuals will be officially registered and listed as delegates and invited to attend all conference events.

  • Recognition during the Conference.

  • Acknowledgement in the onsite program (sponsorship must be paid no later than August 8, 2008 to be published in the onsite program.)

  • Inclusion of a one-page flyer or small promotional item in all attendee packets.

  • Complimentary access to an edited attendee lists three weeks prior to or after the event which may be used for a one-time mailing to those attendees.

  • Listing as a sponsor on pre-show marketing materials (registration brochure), reaching more than 4,500 people. (A signed contract and full payment for the chosen sponsorship must be received by LOMA by June 6, 2008 to be listed in registration brochure).

Refreshment Break Sponsor (Cost: $2,600)

  • Registration for 1 person. Additional personnel can be registered at the rate of $975. The individuals will be officially registered and listed as delegates and invited to attend all conference events. 

  • Recognition during the Conference.

  • Acknowledgement in the onsite program (sponsorship must be paid no later than August 8, 2008 to be published in the onsite program.)

  • Inclusion of a one-page flyer or small promotional item in all attendee packets.

  • Complimentary access to an edited attendee lists three weeks prior to or after the event which may be used for a one-time mailing to those attendees.

  • Listing as a sponsor on pre-show marketing materials (registration brochure), reaching more than 4,500 people. (A signed contract and full payment for the chosen sponsorship must be received by LOMA by June 6, 2008 to be listed in registration brochure).

Sponsorship of Onsite Program ($2,500)

  • Recognition during the Conference.

  • Acknowledgement in the onsite program with an ad on the back cover of the program (sponsorship must be paid no later than August 8, 2008). 

  • Complimentary access to an edited attendee lists three weeks prior to or after the event which may be used for a one-time mailing to those attendees.

  • Listing as a sponsor on pre-show marketing materials (registration brochure), reaching more than 4,500 people. (A signed contract and full payment for the chosen sponsorship must be received by LOMA by June 4, 2008 to be listed in registration brochure).

Conference Bag Sponsorship ($5,000)

  • Recognition of the sponsorship at the event

  • One full conference registration. 

  • Distribution of one piece of collateral in the registration packets

  • Online exposure on the conference website

  • A listing and product description in the onsite material (You must commit no later than August 8, 2008 to be published in the onsite program)

  • One-time use mailing list; your choice, of receiving it either three weeks prior or three weeks following the conference

Conference Materials

Provide us with your branded item(s) and we will stuff them into the attendee bags.  You can provide pens, pads, Post-It Notes, luggage tags or highlighters.  If you have an item that is not listed that you would like to provide, please contact us to discuss directly.  The cost for this is $1,200 for each item that you choose to provide.

CANCELLATION BY EXHIBITOR/SPONSOR
For cancellation on or before August 22, 2008, full refund of monies will be paid.  For cancellation after August 22, 2008 and before September 5, 2008, a fifty percent (50%) refund of monies will be paid.  There will be no refunds after September 5, 2008.

CANCELLATION BY CONFERENCE ORGANIZERS
In the event that the scheduled meeting or event is cancelled by LOMA for any reason, exhibitor/sponsor shall be refunded any Conference registration fees that have been paid.  Under no circumstances, however, shall LOMA be liable to the registrant for any other expenses including, but not limited to, airfare and hotel expenses incurred by the registrant.

SPECIAL ASSISTANCE
In compliance with the Americans with Disabilities Act, all personnel needing special assistance should contact LOMA with their requests in writing.

Electronic Records and Signatures Terms and Conditions

By agreeing to the terms of this agreement and registering as a sponsor or exhibitor, I understand and agree that I am submitting this application for exhibit space/sponsorship by using my electronic signature and that my electronic signature is as legally binding as a manual signature on paper.  These Electronic Records and Signatures Terms and Conditions govern my relationship with LOMA and apply to all communications with LOMA regarding this application and Agreement for exhibit space/sponsorship. 

By agreeing to the terms of this agreement and registering as a sponsor or exhibitor, I agree and consent that information related to this application and Agreement may be provided to me electronically.  I agree and consent to be bound by the terms and conditions of the Agreements, this application, and other related information provided electronically.  I consent to the use of electronic records and electronic signatures in conducting business with LOMA and its service providers, including processing any payment information supplied by me.

I consent to provide LOMA with my current e-mail address and to notify LOMA of any changes to my e-mail address by calling 770.984.6446 or submitting my new e-mail address from my old e-mail address, with an effective date of the change.

I understand that in order to electronically receive agreements, applications, and other information, I will need a compatible operating system, Web browser, and e-mail application, and I will need access to a printer or the ability to download information to keep copies of such documents for my records.  The operating systems, Web browsers, and e-mail applications identified at the end of these Terms and Conditions are compatible.  I furthermore understand that I am indicating that I have the capability to access the agreements, applications, and other information and download or prints copies for my records.  I understand that LOMA’s electronic documents are compatible with the following operating systems, Web browsers, and e-mail applications:

·         Microsoft Internet Explorer 6.0 and higher

·         Adobe Acrobat Reader 6.0 and higher

·         Industry-standard operating systems and e-mail clients

I agree that I may contact LOMA at 770.984.6446 or by e-mail at Insops@loma.org for customer service or technical assistance regarding electronic records or electronic signatures.  I understand and agree that I may obtain a paper copy of the agreements, applications, and other information at anytime by contacting LOMA by telephone at 770.984.6446.  LOMA will/will not charge a fee for any paper copy.

I agree and consent that I may withdraw my consent to receive electronic records and use electronic signatures by contacting LOMA at 770.984.6446.  However, I agree and consent that if I do withdraw my consent, LOMA may cancel my online services or require me to submit a written request for paper copies.

AGREEMENT
The undersigned, on behalf of the Applicant, its employees, agents, and contractors, agrees to comply with the rules set forth in this document by LOMA.  All correspondence will be sent to the person listed above. This document contains the entire agreement between the parties and supersedes any prior agreements. The terms of this document may not be changed except in writing and signed by the parties.
