This application for exhibit space/sponsorship at the Health Underwriting Study Group (the “Conference”) will become an agreement (the “Agreement”) upon acceptance by Life Office Management Association, A Georgia non-profit corporation (“LOMA”).  LOMA will accept appropriate applications on a space available first-come, first-served, basis.  Accepted applicants will receive confirmation e-mail upon acceptance of this application. To ensure the quality of Conference sponsors, LOMA reserves the right to exclude Applicants that in LOMA’s sole discretion deems inappropriate for the Conference.

Sponsorship is not confirmed until LOMA has received a signed application with the agreed upon fees paid in full.  Unpaid sponsorships will be subject to being re-sold.

EXHIBIT OPPORTUNITIES:

SPACE ASSIGNMENTS – Space is limited to tabletop displays or similar sized displays.  Each exhibiting organization will receive:

  • 6-foot skirted table

  • Electrical outlet

  • One-time use mailing list (Your choice - three weeks prior to or three weeks following the event.)

  • Listing in the on-site Conference material

Spaces will be assigned on a first come, first served basis. To ensure the quality of Conference exhibits, LOMA reserves the right to exclude exhibitors that in their sole discretion deems inappropriate for the Conference.

Display applications are not confirmed until LOMA has a signed application. Spaces will be assigned by LOMA to a confirmed applicant (the “Exhibitor”) based on the order in which the applications are received.  Unpaid display spaces after thirty days (no later than April 18, 2008) will be subject to being re-sold to other clients.

Displays will be located in the area immediately adjacent to the primary meeting rooms for the Conference.  While there are no official hours for the exhibits, exhibitors are encouraged to have staff at their display during the following functions and times:

Wednesday, May 14, 2008 (All times are tentative)

Continental Breakfast and Registration: 7:30 – 8:30 a.m.

Refreshment Break: 10:00 – 10:15 a.m.

Lunch in Exhibit Area: Noon – 1:00 p.m. SOLD

Refreshment Break: 3:15 – 3:30 p.m. SOLD

Reception in Exhibit Area: 5:30 – 7:00 p.m. SOLD

Thursday, May 15, 2008 (All times are tentative)

Breakfast: 7:30 – 8:30 a.m. SOLD

Refreshment Break: 9:30 – 9:45 a.m.

Lunch in Exhibit Area: Noon – 1:00 p.m. SOLD

Refreshment Break: 3:00 – 3:30 p.m. SOLD

Friday, May 16, 2008 (All times are tentative)

Continental Breakfast: 7:30 – 8:30 a.m.

Refreshment Break: 10:15 – 10:30 a.m.

It is in the best interest of the Exhibitors to have a representative at their display during these periods.  Each Exhibitor will receive one Conference registration for use by the exhibit staff to attend the entire 3-day Conference.

Displays should be assembled no later than 7 a.m. on Wednesday, May 14, 2008.  Displays may be dismantled and removed at the conclusion of the Conference sessions on Thursday, May 15 or Friday, May 16. 

EXHIBIT/DISPLAY REGISTRATION FEE – $1,125

One complimentary Conference registration is included with each display reserved.  One additional Conference registration is available for $495 and the exhibiting organization may elect to register the additional staff member at any time prior to the Conference.

SPONSOR OPPORTUNITIES:

Sponsors of the below events will automatically receive the following:

Reception Sponsor (Cost: $4,895)  SOLD
Lunch Sponsor (Cost: $4,895)  SOLD

  •       Recognition of the sponsorship at the event with signage and from the Conference podium.
     

  •       An appropriate 15-minute presentation on the Conference program immediately prior to, during or following the sponsored event.

  •       One exhibit space with one electrical outlet - the exhibit area will be host to a lunch and reception Wednesday evening, continental breakfast each morning and refreshment breaks each day.
     

  •       Two full Conference registrations. Additional personnel can be registered at the rate of $495. The individuals will be officially registered and listed as delegates.
     

  •       Distribution of one piece of collateral in the registration packets.
     

  •       A listing and product description in the onsite material (must commit no later than April 18 to be published in the onsite program.)
     

  •       Online exposure on the Conference Web site.
     

  •       One-time use mailing list (Your choice - three weeks prior to or three weeks following the event.)

Breakfast Sponsor (Cost: $2,695)
Refreshment Break Sponsor
(Cost: $1,695)

  •       Recognition of the sponsorship at the event with signage and from the Conference podium.
     

  •       One exhibit space with one electrical outlet - the exhibit area will be host to a lunch and reception Wednesday evening, continental breakfast each morning and refreshment breaks each day.
     

  •       One full Conference registration. Additional personnel can be registered at the rate of $495. The individuals will be officially registered and listed as delegates.

CANCELLATION BY EXHIBITOR/SPONSOR

For cancellation on or before February 15, 2008, full refund of monies will be paid.  For cancellation after February 15, 2008 and before April 18, 2008, a fifty percent (50%) refund of monies will be paid.  There will be no refunds after April 18, 2008.

CANCELLATION BY CONFERENCE ORGANIZERS

In the event that the scheduled meeting or event is cancelled by LOMA for any reason, exhibitor/sponsor shall be refunded any Conference registration fees that have been paid.  Under no circumstances, however, shall LOMA be liable to the registrant for any other expenses including, but not limited to, airfare and hotel expenses incurred by the registrant.

SPECIAL ASSISTANCE

In compliance with the Americans with Disabilities Act, all personnel needing special assistance should contact LOMA with their requests in writing.

Electronic Records and Signatures Terms and Conditions

By agreeing to the terms of this agreement and registering as a sponsor or exhibitor, I understand and agree that I am submitting this application for exhibit space/sponsorship by using my electronic signature and that my electronic signature is legally binding as a manual signature on paper.  These Electronic Records and Signatures Terms and Conditions govern your relationship with LOMA and apply to all communications with LOMA regarding this application and Agreement for exhibit space/sponsorship. 

By agreeing to the terms of this agreement and registering as a sponsor or exhibitor, I agree and consent that information related to this application and Agreement may be provided to me electronically.  I agree and consent to be bound by the terms and conditions of the Agreements, this application, and other related information provided electronically.  I consent to the use of electronic records and electronic signatures in conducting business with LOMA and its service providers, including processing any payment information supplied by me.

I consent to provide LOMA with my current email address and to notify LOMA of any changes to my email address by calling 770.984.6446 or submitting my new email address from my old email address, with an effective date of the change.

I understand that in order to electronically receive agreements, applications, and other information, I will need a compatible operating system, web browser, and email application, and I will need access to a printer or the ability to download information to keep copies of such documents for my records.  The operating systems, web browsers, and email applications identified at the end of these Terms and Conditions are compatible.  I furthermore understand that I am indicating that I have the capability to access the agreements, applications, and other information and download or prints copies for my records.  I understand that LOMA’s electronic documents are compatible with the following operating systems, browsers, and email applications:

·         Microsoft Internet Explorer 6.0 and higher

·         Adobe Acrobat Reader 6.0 and higher

·         Industry-standard operating systems and email clients

I agree that I may contact LOMA at 770.984.6446 or by email at Insops@loma.org for customer service or technical assistance regarding electronic records or electronic signatures.  I understand and agree that I may obtain a paper copy of the agreements, applications, and other information at anytime by contacting LOMA by telephone at 770.984.6446.  LOMA will/will not charge a fee for any paper copy.

I agree and consent that I may withdraw my consent to receive electronic records and use electronic signatures by contacting LOMA at 770.984.6446.  However, I agree and consent that if I do withdraw my consent, LOMA may cancel my online services or require me to submit a written request for paper copies.

AGREEMENT

The undersigned, on behalf of the Applicant, its employees, agents, and contractors, agrees to comply with the rules set forth in this document by LOMA.  All correspondence will be sent to the person listed above. This document contains the entire agreement between the parties and supersedes any prior agreements. The terms of this document may not be changed except in writing and signed by the parties.
