information below and scroll to the bottom of this page to
indicate your acceptance of the terms of this agreement.
This
application for exhibit space/sponsorship at the 2009 LOMA Health
Underwriting Study Group (the “Conference”) will become an
agreement (the “Agreement”) upon acceptance by Life Office
Management Association, a Georgia non-profit corporation (“LOMA”).
LOMA will accept appropriate applications on a space available
first-come, first-served, basis. Accepted
applicants will receive a confirmation e-mail upon acceptance of this
application. To ensure the quality of Conference sponsors, LOMA reserves
the right to exclude Applicants that LOMA, in its sole discretion, deems
inappropriate for the Conference.
Sponsorship
is not confirmed until LOMA has received a signed application with the
agreed upon fees paid in full. Unpaid
sponsorships will be subject to being resold.

SPACE ASSIGNMENTS – Space
is limited to tabletop displays or similar sized displays.
Each exhibiting organization will receive:

Electrical outlet
·
One-time use mailing list (Your choice - three weeks prior to or three
weeks following the event.)

Listing in the on-site Conference material
Displays will be located in the area immediately adjacent
to the primary meeting rooms for the Conference.
While there are no official hours for the exhibits,
exhibitors are encouraged to have staff at their display during
the following functions and times:
Wednesday, April 29, 2009 (All
times are tentative)
Breakfast and Registration: 7:30 – 8:30 a.m.
Refreshments Breaks: 10:00 – 10:15 a.m., 3:15 – 3:30
p.m.
Lunch in Exhibit Area: Noon
– 1:00 p.m.

It is in the best interest of the Exhibitors to have a
representative at their display during these periods.
Each Exhibitor will receive one Conference registration for
use by the exhibit staff to attend the entire 3-day Conference.
EXHIBIT/DISPLAY REGISTRATION FEE – $1,195.
One complimentary Conference registration is included with
each display reserved. One
additional Conference registration is available for $495 and the
exhibiting organization may elect to register the additional staff
member at any time prior to the Conference.

Sponsors of the below events will automatically receive the

Reception
Sponsor (Cost:

Lunch Sponsor (Cost: $4,995)

Recognition of the sponsorship at the event with signage and from
the Conference podium.

An appropriate 15-minute presentation on the Conference
program immediately prior to, during or following the sponsored
event.

One exhibit space with one electrical outlet - the exhibit
area will be host to a lunch and reception Wednesday evening,
continental breakfast each morning and refreshment breaks each
day.

Two full Conference registrations. Additional personnel can
be registered at the rate of $495. The individuals will be
officially registered and listed as delegates.

Distribution of one piece of collateral in the registration
packets.

A listing and product description in the onsite material
(must commit no later than April 18 to be published in the onsite
program.)

CANCELLATION BY EXHIBITOR/SPONSOR
For cancellation on or before February 13, 2009, full
refund of monies will be paid.
For cancellation after February 13, 2009 and before April
10, 2009, a fifty percent (50%) refund of monies will be paid.
There will be no refunds after April 10, 2009.
CANCELLATION BY CONFERENCE ORGANIZERS
In the event that the scheduled meeting or event is
cancelled by LOMA for
any reason, exhibitor/sponsor shall be refunded any Conference
registration fees that have been paid.
Under no circumstances, however, shall LOMA be liable to
the registrant for any other expenses including, but not limited
to, airfare and hotel expenses incurred by the registrant.
SPECIAL ASSISTANCE
In compliance with the Americans with Disabilities Act, all
personnel needing special assistance should contact LOMA with
their requests in writing.
